Patient-Centric Care That Goes Beyond Traditional Medication Delivery
The Pharmacy Challenge
As quick-service dispensaries, traditional retail or community pharmacies are challenged to engage with patients after the handoff of medications. Therefore, patients experiencing multiple comorbid conditions and complex medication regimen are at high risk for errors and readmissions.
The PharmD Live Solution
At PharmD Live, our board certified PharmDs spend quality time with each patient so that we can develop a customized plan and make well-informed recommendations that improve health outcomes, minimize medication errors and reduce costs.
At PharmD Live, we bridge the medication-related gaps between the physician and patient with a focus on preventable medication-related problems that can arise from complex medication therapy. We use new technology, clinical and pharmacy expertise, and patient-centric pharmacy delivery services
